Please read the instructions carefully to be sure you submit all the documents needed for the 2021-22 contract. You can find all documents and resources below. We will continue to update this page as we finalize documents and forms.
New this year signatures for contracts will be completed in docusign. Please review all documents. Once you have reviewed them, please do the following:
1) By Monday, August 23 at noon please complete the fillable sections of the document: Provider_Contract_Durham PreK_2021-2022_fillable
- Return this fillable document to CCSA
- CCSA will then send you a docusign version of the contract to sign at all of the signature spaces.
2) By Friday, August 28 at 5pm please complete the fillable pages in the document: Provider_Contract DPK 2021-22_Attachments D and E.
- Once you complete the fillable pages please return them to CCSA.
- In addiiton to completing the fillable sections, please submit teacher license and degree information with attachments D and E of the completed contracts. You may scan the full document and return it to CCSA.
- Once returned, CCSA will then send you a docusign version of the entire packet that includes Attachments D and E to sign.
Additional Information and Updates:
Programs with more than 1 Durham PreK classroom at the site must complete the following:
- Lead teacher and assistant teacher section of Attachment D for each additional Durham PreK classroom at your site
Attachment E of the Provider Contract should be completed with the seat allocation information previously provided. If you need any clarification about your program's seat allocations or reimbursement rates for the 2021-2022 school year, please reply to this email with your questions.
Durham PreK Teacher Compensation Support Payments will begin in September. Guidance for the supplement is provided in the attached Teacher Compensation Support Policy document.
As was in place last year, Durham PreK sites will also have to complete a 2021-22 Durham PreK Staff Schedule Form for all lead and assistant teachers contracted for the school year. This form, with instructions, will be sent to you in early September.
